I agree with Emily.
If I'm not mistaken, some people from the hospital called Jim “Dr. Clancy” one or two episodes ago… What happened to Dr. Lucas? It's like not only the people who know about Jim have completely forgotten the issue. His own son didn't say anything about hearing his mom and her friends call him “Jim”, but everyone else call him “Sam”. And what about Jim's mom [is she still alive? I forget ><]? Or Sam's family?

I have seen them all and no it is not explained at all. I agree. It should be explained because at the back of my mind I still see Sam and not Jim.

Watch seriers 4, it's pretty much explained in all of season 4. Season 5 is clearly about the kid.

That's irrelevant. They should either find a way to bring Jim back to himself or explain the Sam story away. You can't just ignore it and think it's over now a new series has begun.
