I think Sam's little friend is Lucifer, “think of me as the little fallen angel on your shoulder”. Demons are not ex angels. Demons are hell-spawn, unclean or evil spirits. Fallen Angels, are different and more powerful. Lucifer can be anyone he wants for his purposes. As he was created by God, he would be in God's image, as are we. He wouldn't need a body, as he is not a spirit (because he was never human) since he looks normal. Course she could be one of the lesser fallen angels.
